The welfare reform legislation enacted in 1996 was applauded by many for the successes it had in dramatically reducing the number of people receiving public assistance, most of whom were women with children. Today, however, more than a decade later, these successes seem far less spectacular. Although the total number of welfare recipients has dropped by more than fifty percent nationwide, evidence shows that poverty has actually deepened. Many hardworking women are no better off for having returned to the workplace. In Doing Without, Jane Henrici brings together nine contributions to tell the story of welfare reform from inside the lives of the women who live with it. Cases from Chicago and Boston are combined with a focus on San Antonio from one of the largest multi-city investigations on welfare reform ever undertaken. The contributors argue that the employment opportunities available to poorer women, particularly single mothers and ethnic minorities, are insufficient to lift their families out of poverty. Typically marked by variable hours, inadequate wages, and short-term assignments, both employment and training programs fail to provide stability or the kinds of benefitsÑsuch as health insurance, sick days, and childcare optionsÑthat are necessary to sustain both work and family life. The chapters also examine the challenges that the women who seek assistance, and those who work in public and private agencies to provide it, together must face as they navigate ever-changing requirements and regulations, decipher alterations in Medicaid, and apply for training and education. Contributors urge that the nation should repair the social safety net for women in transition and offer genuine access to jobs with wages that actually meet the cost of living.

|The European Region of WHO has the unenviable position of being the region of the world with the highest levels of alcohol consumption and alcohol related harm. This is a challenge. Relatively simple and inexpensive action can, however, bring rapid and considerable gains in population health and well-being, enhanced employment and productivity, increased health and social welfare savings, greater health and economic equality, and greater social cohesion and inclusion. A public health response is, therefore, feasible and effective. With its first European alcohol action plan in 1992, the European Region has been at the forefront in providing leadership to reduce the harm done by alcohol. Spurred on by the momentum for action brought on by the 2010 global strategy to reduce the harmful use of alcohol, the Region can maintain this world leadership role through the launch of a re-invigorated European action plan to reduce the harmful use of alcohol 2012-2020. The importance of the harmful use of alcohol as a public health priority, with action integral to successful promotion of well-being and healthy lifestyles and reduction of the burden of noncommunicable disease, as well as some communicable diseases, should never be underestimated. Even though only half the world's population drinks alcohol, it is the third leading cause of ill health and premature death globally, after low birth weight and unsafe sex, and greater than tobacco. Alcohol is teratogenic, affecting the developing fetus; neurotoxic, affecting brain development; intoxicating, causing a wide range of intentional and unintentional injuries; carcinogenic, causing a wide range of cancers; an immunosuppressant, increasing the risk of communicable diseases, and a cause of a range of cardiovascular diseases. Alcohol harms people other than the drinker, whether through violence, traffic accidents, domestic violence in the family, or simply using up government resources. The greater the exposure to heavy drinking, the greater the impact on quality of life. Economic efficiency is impaired through diminished productivity due to alcohol. The real risk of dying from an alcohol-related condition increases with the amount of alcohol consumed over a lifetime, with heavy drinking occasions (when most alcohol is drunk) being particularly risky. The action plan was endorsed by 53 European Member States at the Regional Committee for Europe in September 2011 in Baku, Azerbaijan. It includes a wide range of policies and programmes that are relatively easy and cheap to implement, can reduce the harmful use of alcohol, promote health and well-being, improve productivity, and enhance human, health and social capital across the life course from birth to old age. This action plan proposes a range of options for the 10 action areas of the global strategy to reduce the harmful use of alcohol that all European Member States can engage in. With leadership and commitment, all Member States can strengthen their actions on alcohol and subsequently gain from the resulting health and economic rewards.|--P. v.

The springboard for this sixth volume in the Industry and Health Care series was a conference sponsored by the Center for Industry and Health Care of Boston University on June 9 and 10, 1978. That conference had a gradual genesis. Over a year ago we spent some time with Kevin Stokeld of Deere and Company and heard his views on self-insurance and self-administration as one device for a corporation to achieve better management control of its health benefit. More recent discussions with representatives of American Telephone and Telegraph Company and other corporations made it increasingly clear to us that management's need for data to monitor the use of employee health benefits was emerging as a critical policy issue. Subsequent meetings with executives at John Hancock Mutual Life Insurance Company in Boston and Mobil Oil Corporation in New York, among others, convinced us that simple answers would be elusive or inadequate and that there was a need for an objective and careful look at the evolving relationships between employee health benefits, claims administration, health services utilization, and corpo rate health care cost containment programs. Since self-funding and particularly self-administration represent a fun damental change in the traditional insurance relationship, the conference was convened to explore the advantages and disadvantages of self-insurance for employee health benefits, with some attention to claims production but with special emphasis on the originating question of data for effective management of an employee health benefit.
